Due to requiring a range of clinical information for our group exercise classes, please take the time to provide responses to the following questions. This is to confirm that participants selected for the class meet our requested criteria.
To ensure that participants for the classes are selected fairly, all EOI submissions will be assessed individually to identify which participants have meet the requirements of the EOI and are most suitable to attend the class.
Please note, we are unable to keep EOIs on file as a waitlist for future programs. All classes require individual EOIs to be submitted.
If you wish to join multiple classes in a region, you will be required to complete separate EOI forms for each class.
