The purpose of this health declaration and online check-in form is determine whether you are at risk of contracting or currently have signed/symptoms of COVID-19 and to collect the required data for contact tracing.

You are required to complete this form to participate in any face-to face Mates4Mates activities. This is in the interest of ensuring Mates4Mates is a COVID Safe Organisation.
