Mates4Mates Diabetes Management Program

Are you a veteran or family member who wants to improve the way you manage your diabetes condition? 

Then the Diabetes Management Program may be for you.

What is the Diabetes Management Program?

Newly developed by Mates4Mates, the Diabetes Management Program is designed by our exercise physiologists to help veterans increase their confidence to self-manage their diabetes with health interventions, while building their strength and fitness to improve their blood glucose control.

The group program will provide an opportunity for participants to become better educated on diabetes comorbidities while also learning behaviour change strategies.

The Diabetes Management Program will be offered at both the Brisbane and Townsville Family Recovery Centres. 

An initial consultation will take place prior to the program, with a final consultation to take place after the program is completed. 

This program will require participants to have a DVA or Medicare referral. 

Upcoming Programs

This group program will run on Tuesdays and Thursdays, starting 2 May to 8 June 2023, from 1:00pm to 2:00pm.

The program will be held at both the Brisbane Family Recovery Centre and Townsville Family Recovery Centre. You will be asked to select which location you’re wishing to attend within the below expression of interest form.

Expressions of interest close Monday, 3 April 2023, however, expressions of interest may close sooner if a large volume of applications are received. It is therefore recommended that expressions of interest are submitted as soon as possible.

Places in the program are dependent on meeting the selection criteria. Please note submitting an expression of interest does not automatically qualify an applicant for the next step in the selection process for this program.

Places in the program are available to veterans and family members registered with Mates4Mates.

To ensure that participants for the program are selected fairly, all EOI submissions will be assessed individually to identify which participants have met the requirements of the EOI and are most suitable to attend the program.

Please note, we are unable to keep EOIs on file as a waitlist for future programs. All programs require individual EOIs to be submitted.

Please note: dependent on the program you are applying for, a clinician may ask you during the screening about the possibility of you obtaining a referral for the program. While a referral is not a requirement to participate in the program, it is recommended.
